Referee Report Card
As a League appointed Referee you can use this online form to submit your Referee’s Match Report & Assistant Referee’s Award information.
Simply fill in all the details of the match and then press the "Submit this Report" button found at the bottom of the page. When you submit the form
an email will be sent to 'ref-liaison@mk-youth-football-league.org.uk' and a copy will be sent the email address(es) listed in the 'Email copy to' field.
Use a comma to seperate email addresses, e.g. "u13mgr@myclub.com, clubsec@myclub.com"
This form will try to use cookies to remember Referee names and email addresses. If more than one Referree uses your PC to submit their
reports then you can recall the information for a specific Referee by selecting a Referee from the list presented at the bottom of this form.
