Fairplay and Match Result Sheet

As an Administrator for one or more teams you can use this online form to submit your Fairplay and Match result sheet information. Simply fill in all the details of the match and then press the "Submit Match Report" button found at the bottom of the page. When you submit the form an email will be sent to 'results@mk-youth-football-league.org.uk' and a copy will be sent to you.

Mark each section out of 10. The administration mark will be ignored for home team submissions. If any section is marked 3 or less then a letter of explanation must be sent to the Fair Play Secretary within four days of the match.

Enter the TOTAL fees (match and mileage combined) to League officals like this; £15:00 or £19:50. Enter £0.00 if you did not pay anything.

5) Teamsheet

For League and Cup matches there is an FA requirement that you submit a teamsheet with the list of players who started or came on as subs. You should update and save a copy of the League provided Blank Teamsheet Word document then use the Browse buttom to locate it on your computer.
